Child born in UK, Kindly need help please

Dear all,

Please could you help me finding the correct application form for my child born in the UK (applying from UK) to apply for VISA.

My original visa: HSMP - Aug 2006
Extension : Tier 1- Aug 2009

I am not sure if I have to use FLR(Tier 1) HSMP or PBS Dependent application form.
