Confusion On FLR (M)

Having some problems with the form. It is a application for a extension to further leave to remain for another 2.5 years. It asks in 2nd section if this is the first time we are applying for a visa or extension for a spouse visa would that be yes or no since we have applied once before for FLR (M) and was granted it for the first 2.5 years.

Secondly is is ask if this is the second period to remain that is a yes.

Another bit i am having a problem on is immigration History, it asks if she entered the UK Legally which is a Yes but on the date there is confusion.

Do i put the date when she originally came into the country or the date she was given FLR (M)? She came in the country on 12th April 2011 on a spouse visa married to a previous partner but that marriage ended in divorce and she then married me and we applied for a visa and got on on the basis of marriage, do i put the date she came in to the UK on or date she was granted a visa with me?

Also it asks if i am applying for first extension but this would be a second extension right?
