Consent Form for PBS dependent Extension

Hi,
I just got my ILR and we are applying for my wife's extension as Tier 2 PBS dependent.
In addition to proof of co-habitation, Maintenance funds, passport/visa copy, ILR copy, Checklist. Are there any other documents which should be submitted? For eg, i had to sign and submit a Consent form. Is there anything like this for PBS dependent extension.
Visa appt is tomorrow morning so a quick feedback would be appreciated.
