Dependant of Tier 1 - Previous Name

Hi all,

I wish to extend my Tier 1 dependant visa and have a query with regards to my previous name.
This seems to be a new field in the form where the dependant has to provide details of the names he was previously known as.

As per all my passports the old name was never used except in the old passport, there is an observation added in one of the pages which states "The holder of this passport was previously known as xyz"

Now since i have to mention this in my form what is the evidence that I can provide. This name was used only from 1977 to 1988 and after that all my certificates & all my documents show the new name.

Note: The change in name involves removing an additional Village name prefix with my first name.

Will this cause an issue with the extension?

Any inputs from the gurus would be highly appreciated?
