dependants name in AIT form

HI,
as i hv to appeal to AIT b-coz my application was refused on the basis of maintenance under transitional arrangements, is it possible to write Dependants names in the main applicant's AIT 1 form or they have to fill their saperate forms for that?
have to send documents tomorrow
