Dependent visa for Tier 1 spouse

Dear members,

I was under the company sponsored work permit from October 2005-September 2010.
My company has now moved me to tier 2 ICT established staff visa which has given me leave to remain till September 2012.

My wife was on separate work permit from March 2006-August 2008. She then moved to a Tier 1 visa (formerly called HSMP) from August 2008. It expires in August 2011.

I want to transfer my wife to my dependent visa when we visit India in December. The rationale for that is that we both can apply for ILR together in January 2011.

I am wondering what is the right form to be filled.
I have this form Application form VAF2 - Employment

Dependent(s) of PBS migrants need to use PBS forms!!
