My question;
1. in section 12b, the employer's declaration, he has to sign his name himself due to he is only one director in the company. Is it sound fine?
2. Do I have to ask the accountant to write a letter to confirm his company and his status? and may I get some idea about the letter, please.
3. In document required in application guideline, HO mention only the payslip and bank statement. Do I have to provide them extra docs i.e. P60, tax assessment or any recommend docs, please?

Thank you very much for your advice. I am now still confused because my spouse should be worker section because he pays tax on a basis of employed. For self-employed section, there is a question asking when the spouse registered as self-employed. thanks for your comments. I have read all docs provided from HO website but still confuse that why I am here searching for help. For HMRC, my spouse is employed and always fill up form of tax return as an employed the not self-employed. He has both salary and dividend voucher. all these make me so confused. Why don't you read a full guidance notes from home office for self-employed person.
As a sole director of a company your spouse may get a dividend not a payslip.
But if your spouse is getting a payslip then your spouse still needs to fill a tax return as a self-employed person

Why don't you read a full guidance notes from home office for self-employed person.
As a sole director of a company your spouse may get a dividend not a payslip.
But if your spouse is getting a payslip then your spouse still needs to fill a tax return as a self-employed person
