Disable Dependent- Tier1 General

HI Fellows,

I have tier 1(General) from 2011, I want to apply for my wife and kids visa from Pakistan.
My wife is disable. I have four kids.

Could you please advise which documents i need to submit application.
There is any special requirements to apply for disable dependent visa.
