Dividend vouchers

Can Moderators/Seniors/Gurus help me on this.

1. Should I use net or gross amount for calculating my points with dividend vouchers.

2. My dividend vouchers are formal but on plain paper and stamped. Should they be on Company letterhead.

Reasonable help will be highly appreciated.
