Document certifying PR

Merely out of interest; which form does the Document certifying Permanent Residence for EEA Nationals take? Does it come as a sticker in the passport or as a separate document?
If it does come as a separate document, could this be used to enter the UK on its own or only in conjunction with a valid ID card/passport? (when one goes by car/train of course; I can imagine that Stupid probably wouldn't let you travel with it.)

It comes as a separate document as EEA regulations do not allow for endorsement of EEA passports with any form of stamps/vignettes. Technically, it serves very few purposes except for naturalisation. You would be able to rely solely on your EEA documents to enter and reside in UK otherwise, including employment rights, benefits, etc. Do not consider this as a widely circulated document; in fact, it confuses a bunch of public servants.
