documents for spouse ILR

After a year of waiting, I finally received my ILR BRP. Huge thank you to many here, especially Amber.
I need some help with my wife's ILR application. I searched the forum, but still no sure.
1. My wife has been a Tier 2 dependent since 2009. She got her first Tier 2 dependent visa in 2009, second one in 2013.
In this case, does she need to provide cobabitation evidence for the last two years or five years?
2. she can use SET O form?
3. I went through SET O form, there is no requirement of sending my document(e.g. BRP) as well. Is there a need to send my BRP or provide my BRP number for my wife's ILR application?

Any reply will be greatly appreciated.

I settled as a Tier 2 migrant.

SET(O), B1 English and LIUK required, a certified copy of your BRP and Passport, in addition to her originals and 2 years cohabitation proof.
