I would be very happy if someone could create one for under 28. Anyway you're lucky guys (over 28) ;)

I will give the under 28 form a try---hopefully it will be up in another 24 hrs

Form Does Not Save info but will let you fill and print if you have Acrobat Reader Only

But if you have acrobat reader writer then you can save info and print the form

I opened the pdf file, one page at a time, in Photoshop Elements. Just make sure that you don't flatten the layers, if you think you might be making any changes at all. Otherwise, you have to redo the whole page to change anything, which is a pain.

I tried that...it saves....but gives me back your sample info when i re-open the doc !!

1. Try using "save as" option.
Or
2. Use "Adobe Acrobat" instead of "Adobe Acrobat Reader"

I got it. My Acrobat Writer had expired. I re-installed its saving now.
