EEA family visa expiring soon

Hi all,

A friend of mine is european, and his wife is currently on her 5th and final year of her EEA family visa (it expires in 13th march 2011).
1- What is the right application form to fill in?
2- does she have to apply for Life in the UK test?
3- She is a housewife (looking after her children) which means she did not work in the last 5 years, but they are self sufficient as the husband work full time. Will this cause her any problems?

Any advice would be appreciated.

2- No.
3- No. The European national is obviously a worker and thus not classified as a self-sufficient person.
