Employer didn't pay the minimum for the past 2 years

Hi, I got a General Employment Permit almost 2 years ago, but my boss didn't end up paying the minimum salary, or even hourly, but just for whatever work came in for me. I largely lived on credit cards and selling my things. Now, I can't prove my income or taxes for a renewal. What would be the best way for me to continue?

(1) If I quit and get a new job, will DETE approve my new permit on its own, or will it ask me for proof of the previous permit's income?
(2) If my present boss makes me redundant and I get a new job, will DETE approve my new permit on its own, or will it ask me for proof of the previous permit's income?
(3) Or is a Reactivation Permit the only option?
