Employer Letter format for previous earnings as a proof

Hi

Im going to apply for HSMP in March first week. For previuos earnings document , i have last 12-months payslips ONLINE .

In order to authentiate these pay slips, i need to get the HR sign and stamp from my company.

But my HR is not willing to give sign and stamp on the payslips .

But he is ready to give employer letter for the previous earnings . So what is the format for that letter or can anyone give me the same letter pls
