employment histroy details missing.

I am about to apply for my naturalization but need help filling the application on following two issues related to employment history .
1- As i have to mention the exact date and year of my first job as but the problem i do not remember the actual start date although i am sure i started working in 2007.I called my branch and as per records my start date with them was from November 2008 until march 2015. I even requested hmrc to know the exact date but from past 4 month no reply from them either?

Now i am confused which dates and year should i mention ?

2- I applied for HSMP on the basis of self employed in 2009 (registered as self-employed in 2008) but since then I haven’t used self-employed option but I am still registered as a self-employed. I got extension and ILR as director of a LTD company and working part-time at Tesco. Recently I have been working as PAYE from past couple of months.

My question should i mention as a self-employed like ‘Self-Employed from 2008 to 2009 or 2008 to date’.?
