Extension of Tier 2 (Senior health Care Assistant) Visa

My current tier 2 Visa as Senior Health Care Assistant will expire in January 2011. I am working as a Senior HEalth Care Assistant in a Nursing Home.

Now, for extension, must I extend through the same current employer or Can I apply through the Certificate of Sponsorship from another employer?

Also, will the new rules apply to me or the old rules (at the time I first applied for the Tier 2 Visa) will apply?

Please suggest me..
