FLR (M) Queries

Dear All please answer following few questions in helping me to help my friend:

1- Dependent of some one can use form FLR (M), if main applicant has applied for ILR on long stay basis (10 years stay) and main applicant case is yet not decided?
2- The financial requirement of £18600 or what ever, is required for any twelve months or should be of UK financial years like starting 6th April of one year and ending 5th April of next year?
3- The maintenance requirement can be a combination of salary and self employment of main applicant (Sponsor) and the dependent that is applying on the form FLR (M)?
