Format for Letter from lawyer for thirdparty funds

Hi all,

Can someone tell me where I can find a format for the letter from the lawyer for the verification of signature of thirdparty.
I could not find it in the guidance although it mentioned that its present in the end of the document.

Any help will be greatly appreciated.
