Forms for Tier 1 extension & spouse’s dependant extens

Hi,

I am holding an HSW visa (Details given below).

Visa type : HSW tier 1 general
Issue date : 30th Oct 2008
Expiry date : 30th Oct 2011

I will be applying for an extension somewhere in 1st week of Sept 2011. Along with me I am applying for my spouse’s dependent extension too.

I am filling up the following forms

For main applicant : Version 04/11 (Application for an initial grant of leave (switching) or an extension of leave and biometric residence permit under tier 1 (general) main applicant)
For Spouse’s dependant : Version 07/2011 (Application for an extension of stay as a dependant of a person who has leave to remain under the points based system)

Am I looking at the right forms?
