guidance needed for new born daughter's visa

I came to United Kingdom in 2005 under old HSMP rules.

My wife joined me later as dependent under new tier 1 dependent category to UK and now we have been blessed with new born baby girl here in UK.

I request you to guide me as regards following queries :

1. What/ Which Visa form / category should we use to apply for my
daughters visa?
2. What are the documents required to apply and get her visa.?
3. how long does it take to get my daughter's visa stamped from home
office and on which home office address i send all relevant
documents?
4. What will be the visa fees ?
5. Is there any speed / faster procees available to get same day visa/
clearance for my new born daughter?
6. I am from india and applying for indian passport for my daughter as we are planning a short trip to india soon, i came to know from my friend i need to get register the baby birth FIRST to indian high comission in holborn and then secondly apply for passport !!, now if someone did this before, my query is do i need to do two things or it is ok to apply for passport directly with birth certificate, my passport and wife's passport and other relevant document? or do i need to get FIRST birth register with indian high commission and then apply for passport?? ( i dont like the long queue outside started from 6 am in the morning to get 100 tokens, that is why asking this question!!)

Thank you for your reply.

Further, i got time off from my job due to pregnancy complications as well as recent financial market turmoils,,.... whether it would affect the outcome of the application of my daughter?

Is it appropriate to apply from india as my wife is planning to visit india at a later date, i.e. by that time i will get a job and can produce payslip, bank statements etc

OR - submit application here in uk with bank statement, tenancy agreement and previous employment payslip ( last one is till nov 2008)??
