Dear, Every one who ever can help

I want to start with VAF5 form Do I need to fill part five I am the husband of EEA Natioanl when I go to part five it stated :

To be completed by

extended family members of an EEA national applying on the basis that they are depend on that EEA natioanl ( As far as I understand as her husband I do not fall in this list) Please correct me if I understand it wrong or

dependent direct family members ( this does not include spouses, civil partners or children under the age of 21) according to my understanding that I am a spouse so I leave part five. ( Did I fill this form right?

Now here is the thing about my wife permanent address she lives with me but can I show her mother address her permanent address and my address her correspondence address will it be ok?

I have proves that my internet is on her name and we get the bill I have the contract with her name too but I have a court case going on so I have submitted my original contracts and I forgot to keep the copy I will explain to them in additional information.

I have another problem is that I have a residency card which is stated date is that 28 feb 2014 valid but this card has been block by the EU country in which I am staying but that is why I have did this case which I will also going to explain in part 9 additional information.

and I have lived in UK for about 7 years before and and I have visited many other countries and I have no other immigration history problem in the past.

I want to know what are the chances of me getting the EEA family permit?

If some one can help please address any of my issues which I have mention above.
