Help to complete some part of the Tier 1 Application- Query

I am in the process of applying for Initial Entry (Swicthing) with in Teir 1 (General) Main application. I am in part-way process of HSMP and I have valid HSMP Approval letter with me, so as per the new policy I am filling the Tier 1 application form. I am currently in UK on Workpermit for the past 4 years 1 month. I need to clarify some questions before providing the details in this form.

1) In Page 6- Section 2 --> Question B10. Home Office reference number(s) (if known)

There are two boxes. Which Home Office Reference Number should I fill in those boxes. Is it the work permit reference number or the reference number provided on the HSMP Approved letter?

2) IN Page 9 - Section 2 ---> Question C6.

I have filled my passport details. Do I need to fill the passport details of my dependent(Wife)?

3) In Page 10 of Section 2 --> Under D - IMMIGRATION HISTORY there a two question D3 and D5 which needs clarification.

D3 - Since I have the work permit and renewed twice from the date of entry in UK. Which date am I suppose to provide for the question 'When was the entry clearance or Visa issued?'. Is this the last latest renewal or is the first one when I was coming to UK?

4) D5- Again based on above facts, the question asks 'Please enter the visa number on the entry clearance or visa number within the application's passport or travel document', so which visa number should I provide?? Is it the first time when I came to UK or latest renewed number available in passport?

5) In Section 5 - SUMMARY SHEET

Do I need to give all the proofs for the points scored or do I need to fill against only the funds maintenance parameter for 10 points??

Kindly clarify the same at the earliest.
