You have to complete 'PBS Appendix 1 General Migrant self assessment form'. So, the number is 1 and the title 'General Migrant self assessment form'

Mention that the passport is damaged and where it is now, if you still have it enclose it and mention 'enclosed' or if surrendered mention surrendered to whomever your surrendered your passport.

if you dont have one leave it blank
