Help for HSMP

Hi
I want to apply for HSMP under Tier1. I have some question, Please anyone help me , I am from Pakistan, over 32 years, I have a Master degree in Computer Science which can give me 35 points. Now I need 40 points under the earnings. For Pakistan, I need 7500 Pounds earnings (141 * 7500 = 1057500 PKR), that can give me 40 points then it will be 75 points total to qualify HSMP.
During last 12 months I have earned Rs. 440000 from my Job, Payslips are available and all this amout was transfered into my bank account and is shown in statement. Rs. 440000 is gross salary. Net Salary transfered into my account which is obviously less then Rs. 440000 (Provident fund, tax and other deductioned were made from my gross salary, all these deductions are mentioned in my payslips.)

Question 1: Can I claim Rs. 440000 as salary or I can clain only the amount that is transfered after deductions??

I am IT professional and I do some parttime software development, I have made some projects for various companies that is amounting Rs. 622000. This amount is also shown in my bank statement in bits and peices. I can have cotract agreements for which I worked for but all the contracts were verbal.

Question 2: Is is OK to include Rs. 622000 in my earnings. What should the contract letters show for this type of earnings.

Question 3: Rs. 1062000 is OK to quaify for HSMP.

Question 4: I have not yet appeared in IELTS, but I can pass that as well, I have good communication skills. I have got a letter from my university stating that the medium of instruction was English for my Masters program, is it sufficient to claim my english language skills? or do I need to have IELTS as well.

Question 5: How long it will take to process my application.

Question 6: Whats the fee for HSMP application. (for my and my wife).

Question 7: Where can I find the application form.

Question 8: How much funds are requried to be shown in my bank statement before applying HSMP.
