Help AN Form Paper Application through NCS

I have booked an appointment in December 2018 in a local Council NCS for Naturalisation application. Filling up the form I have got some confusing question. Please somebody known or faced these issue can help me.

1.23 Please Provide Details of Your English Language Test
If I am not wrong, can I leave it BLANK as I do not have one, But I have a UK Degree and I have done Life in the UK Test. So I will submit my documents along with AN Form. is that okay?

1.49 Tax reference Number. what would that be? Please help. I have checked my current payslips..there is none.. I have checked my old p45..I dont know which one that could be? If it's employer PAYE Reference, if so then how could I get my current employer PAYE Reference. I have got old p45 where I can see two numbers.. 1) Office Number 3 digit and 2) Reference Number H123.

1.50 If I do not remember and do not have all p60 or payslips what can I do in Employment History? Can I leave this boxes BLANK? or whatever I remember can i put them and also I do not remember exact date of start and end the job? HELP

Section 5 - Referees... Can I give a reference from my GP Doctor?

6.7 BRP Enclosed? yes or no? if yes, should I send it through the application? If NOT enclosed, then state the location of biometric residence permit: what to write in this box?

6.12 Date your fingerprints were taken? what fingerprints? I yet did not give any fingerprint.

6.14 Give details where your fingerprints were taken, including the town city and country; ?

6.15 Give details of the British diplomatic post(s) involved if the application(s) was or were made abroad: what to write in this box?
