Help needed to fill the form

Hi,

Currently, I am in UK and planning to apply for Tier 1 in the last week of this month. I need help in filling my application form and listed my doubts below
  • B11 -> Worker reference no - Where will I find it? Is it 'Our reference number' in work permit?
  • D2 -> I travelled to UK on two different work permits in the last one year. I believe I should fill the details of latest VISA. I request you to kindly confirm
  • D3 -> Date for Entry Clearance -> Is it VISA 'Valid From' date on the passport or Work Permit Date?
  • D4 -> Reference Number -> Is it VAF No in the VISA? Please confirm
  • L7 - Previous Earnings -> For the last financial year, I worked in India for 4 months and including the period for previous earnings. In L7, do I need to enter equivalent earnings in Pound i.e) after using the uplift ratio or just earnings in pounds?. Also, I believe, in column 'Evidence of Earnings Provided', do I need to list the evidences or just say 'Yes'? Please confirm.

i think you should use Uplift Ratio.
