HSMP Dependant Visa from Pakistan

Hello,
I applied for HSMP from UK and got my HSMP approval letter on October '07 , After this i sent my passport Visa stamping on the basis of my HSMP Approval letter on November'07 . I was granted 2 year HSMP Visa.

I now desire to bring my Wife (Who is in India at the moment) to UK as my dependant . She had been to VFS office in India and it looks like they are confused on the following issues

1) Which form she has to fill up (i.e. VAF2/VAF10)
2) What should be the fees of her application (i.e. 29200 PKR/90000 PKR)

Can somebody please help me out here .
