HSMP extension & Limited Company

Is there anyone there who can provide the requirements for HSMP extension for a self employed person with his own limited company?

Details:

Granted HSMP in December 2007
Established own limited company
Pays self some salary and dividends

What document needs to be submitted for extension, please?

Any advise would be appreciated.

Application form suggests four -2needed

dividend voucher
accounts
income statement
sales summary

You can't be self-employed with ur own limited company, unless u have a ltd u trade thru, and u work SE too for other work. I suspect u r an IT contractor tho....
