HSMP Initial Apply - what else documents should be sent

Hi All,

I am planing to apply for HSMP, could any one please tell what else documents I need to send along with the supporting evidences for my points claim. (eg: do I need to send passport photocopy?)

And also do you recommend any particular order of packing the application to send it to HSMP office?
