I am applying from Pakistan for HSMP (UK) , is it necessary to appear in IELTS to prove english proficiency, as an alternate what other evidences can be supplied which are must ?

Not necessary. You can show any other course certificates. You can also state for example education in English, day by day work in english. This is not qualifying criteria and they would not refuse you on this basis.
