ilr dependant less than 2 years

hi im goin to apply for ilr(tier 1 general . 5 year) at peo croydon with my wife this 30th april 2014.
i know i have to use set(o) application.
my question is what application do i need to use for my wife?? as she first came to uk in dec 2012. and not making 2 years.
is it just set(O) .?? or flr (m)
as i already booked the appointment for both at same time, dependant for my wife.
i dont knw what to do??
plzz any senior member help me out.

When was she first granted PBS dep leave, what date?
