ILR Documents

Hi Friends, need advice.

I am planning to apply for my ILR in two weeks and would appreciate your feedback. I came to UK in 2003 as a Holiday maker and changed to WP in 2005. I had to go to my country to get my WP ( entry clearance) and obtained this in August 2005 and entered UK in August 2005

a. WP - entered UK as a WP holder in August 2005
b. Changed to a different Company in October 2005
c. Converted to HSMP in October 2008
d. Will be completing 5 years WP + HSMP in August and would like to apply before the 28 days
e. Passed the Life in the UK test.

My question is in regards to documents. I am planning to provide the following,
1. Work Permit for the first company - Original
2. Work permit for the 2nd Company - I only have the copy not the original ( will it be a problem???)
3. Last 6 month's payslips
4. Last 6 month's Bank Statements - Personal ( don't have a big balance?)
5. Employer Letter
6. Last 3 month's business bank statements ( the Turnover is less than 2k?)
7. May be my last Self Assessment Return - is it necessary?

Have I missed anything? Please advise.
