ILR Guidance

I and my two sons have been dependent on my husband visa, initially we were dependent on his tier 2 visa and then for last 2 years on his ILR status. In total we have completed 5 years as dependent. I have cleared my life in UK test and want to apply for ILR status with my two sons. Please advise which form we (I and my two sons) need fill.

What was your visa type in the last two years ?
Did you switch to the family route using form FLR(M) ??
