ILR WP - Set O - Appt @Liverpool PEO - Few quetions

I have my appointment booked for ILR application for 26th Oct 2011.
I have a few questions/doubts.
1. I entered UK on a permit free immigration as a Student Nurse in March 2005 and Switched to WP in 2006, now the application asks the date I entered UK do i fill in the first arrival date or the date when my WP (LLR) was stamped?
2. My husband (dependent) has a biometric LLR as dependent that he got when he sent his paspports for the visa to be transfered to new passport and was issued a biometric card - so can he apply for ILR on the same application form?
