Cohabitation Documents FLR(M) - Unmarried(Extension)

Thank you for the support you all are giving in this forum. I would need your input and guideline for my query.

I am planning to apply for FLR(M)[Unmarried] in November, the expiry date of the current is in 15th of December,i was on dependent visa(2015) based on our 2 British children and switch for unmarried FLR(M) in July 2017. I had a query about the cohabitation documents. We don't have joint bills/account in our name together, all the documents are separate.

I got my current FLR(M) in July 2017 while we were living in a home rented by family and in December 2017 we moved to my new home(Mortgage).

These are my following queries below for Cohabitation Documents:

1- Do i need to provide 2.5 or 2 years Cohabitation Documents for the extension of the FLR(M)[Unmarried]?

2.1- Barclays - From[July 2017-Dec2017(previous address) - Jan 2018 till date(new address)]
2.2- TSB -From[same as above-2.1]
2.3- Natwest Saving(Wages paid in) - From[same as above-2.1]

Could you please check all the above are suitable/missing/overloading for the extension of FLR(M). Or could you specify with of the above are suitable. My income is over 45k as an employee and i have been working with the current company for 20 months.

4. Do i need to provide 12 months payslips?
