I submitted biodata and any used pages for both current and previous passports just to be on the safe side and had no issues. However, I don't think submission of previous passports is a requirement for a visa extension and they should have this information anyway from previous applications so I imagine just submitting biodata page for current passport and any pages used since the last visa granted would be fine as well. The documents checklist generated at the end of the application is probably the best guide of what must be submitted.

Cover letter should be fine. I don't think these questions are particularly important unless you are concerned you would not meet the requirements for the visa in which case these questions and the evidence provided would be used in consideration under other visa routes.

If you have joint accounts, then both you and your husband will have to sign a declaration. I believe third party is intended to mean anyone other than you and your partner (like parents, children, etc.) in which case they would also have to sign declarations. The required declarations can be downloaded at the end of the application and uploaded with the supporting documents.

Don't worry too much about where specific documents get uploaded as the category choices are very poorly worded, choose what you think is the most relevant category. The categories are generic and some are not applicable for most spouse visa applications (like medical information). Just make sure everything that should be uploaded gets there even if that means lots of documents in the "others" category. I put things like my marriage certificate in life events, employment information should go in employment, financial (bank statements) in finances, cohabitation evidence could probably go in residence or others.
