Form selection for dependent visa

Hi I Have received UK ILR ( on basis Long term residence)
I want to apply for my wife visa
She has previously Tier2 dependent visa and she also working in UK.
Now she want to apply for visa. My question is which application Form I have to use
FLR(M)
Or
FLR(FP)
