Indefinite Leave to remain

Can anyone please guide me about the form to be used for submitting an ILR application to home office in following case:
Applicant entered the United Kingdom alongwith his father on dependant visa to join his mother who is british citizen.At the time of entering the UK the applicant was under 18.However now he is over 18 and has to apply for indefinite leave to remain next month.
Can anyomne please suggest the form to be used and the document which will need to be submitted.
