Application form for Tier 1-General PBS Dependent ILR application

Hi All,

Apologies if this is a duplicate topic, i couldn't find answers to some of my questions related to the topic.

I am a British citizen (through the Tier 1 general route), my wife is on the tier 1 dependent visa and I need to apply her ILR now. (on the 5 year route as she came to UK in dec 2012)

Is it the Set (M) or Set (O) application form I need to use?

I have got the below supporting docs for the application, is this sufficient or do I need to add anything else ?

1.Life in the UK Test pass notification
2.Marriage certificate
3.My personal bank statements - 6 months
4.My company account bank statement - 6 months
5.Company accounts statement from my accountant
6. UK naaric letter to meet english language requirements

Please can some one advise on the above.
