There is nothing wrong in sending a cover letter just to give clarity to any thing you think the CW might struggle to understand, it’s not out of place.

According to the guidance you must mention all wage rates during the period of claim. Its mandatory.

“(c) Confirmation of the employment start date, job title, job description, hours paid per pay period and the hourly rate for each settled worker relied upon to claim points, including any changes in the hours worked per pay period or the hourly rate and the dates of those changes”

Job creation is the major part at ILR stage. I would urge you to clearly mention how you meet the Job Creation in the cover letter so its easy for CW.
Also, its up to you if you want to add other pointers also.

I haven't filled the online form but I would think it already caters for multiple wage rates. It is no different to entering different employees under the same job. You should be able to enter start date and end date for each wage rate under the job with the rest of the employee details (name, start date of job) being the same for each entry (as though each wage rate was a different employee)

In the ILR section, it does say that the evidence requirements of the extension applies to ILR application.

Lets say employee A was hired from 1st Jan 2018 till 31st Jan 2018.
1st Jan 2018 till 31st March 2018 employee A was paid £7.50

1st April 2018 till 31st Dec 2018 employee A was paid
£8.50

The online form first asks about the JOB title and then the list of employees who fulfilled that JOB for 12 months. In the above scenario its only employee A.

So there are two options:

Option A: List the same employee A twice under the same JOB for two different pay periods.

or

Option B

List the current JOB rate on the online form and list the previous wage rate and specific dates in the cover letter.

I personally did option B for my application.

Scenario
Employee A Jan'19 to March'19 pay rate £13hr for 130 hours then March 19 to May'19 £10hr for 160 hours then again changed to £13hr for 130 hours till he left.

So I am gonna mention the rate in the form when the employee A left which is £13hr. Is that ok and will mention the dates of rate changes in cover letter as you did. Hope this should be ok ?
