Intending to visit UK for Tier-1 extension

I got my Tier-1 visa in March 2009 and after that I only visited UK once for 10 days time period. My current visa is valid until March 2012. I want to visit UK this year in November and want to extend the visa after booking an appointment. Can anybody help me with the following questions:

1.I have not lived in UK after getting the Tier-1 visa. However, I do have the minimum qualifying points based on my income and qualification, even without taking the cost of living ratio multiplier in account. Do I have to be permanently living in UK at the time of the renewal of the visa? I did not see any such mention on the UKBA website though.

2.If I extend the visa before three or four months of its expiry date. Do I get another visa starting from the date it is stamped or from the expiry of the first Tier 1 visa?

3.What form do I need for myself, my wife, and my daughter to apply for extension?
