End Civil Partnership

Hey guys,

This might seem off topic but I need to end my civil partnership of 6 years in order to apply for my citizenship.
I been to the family law court and they said that the form 2S that I submitted was missing information. They told me to add Applicant Claikd and showed me a quick example where the person added a lot of paragraphs.

Any idea or suggestions?

Anyone has been thorough this and can give me a template to follow?
