Please excuse my ignornance everyone. I’m still waiting on my FBR, should be coming reasonably soon (next couple of months hopefully). Can anyone advise me on what I need for my passport? I’m living in the US, my Grandmother was Irish. From what I can tell I need My birth certificate, my passport, my marriage licence, a couple of utility bills and an idenity verification form (anyone know what this is?). If I’ve missed anything please let me know and once again please excuse my ignorance.
Identity Verification Form is for your witness (like in FBR application).You'll apply online & need a digital photo (EU size) to upload when you apply. You'll need an ID -if you have a passport send a notarized color copy. You also send a notarized color copy of your FBR certificate. Here is stuff from DFA.ie if passport via FBR first time passport applicant.

Identity verification form.
Original FBR Certificate or a certified colour copy (certified by a solicitor or notary public).
Full original civil birth certificate (version that includes the name(s) of the parent(s)).
Original Civil marriage or Civil Partnership certificate (if applying for a passport in your married name).
Proof of address such as original government correspondence such as letter from the local government health services, local social welfare services or other local government services. These must be accompanied by a translation into English or Irish. Where you do not have government correspondence, original utility bills, or bank statements may be considered.

You must submit one document as proof of name and one document as proof of address. It cannot be the same document.

*Copies must be certified by a Garda, Solicitor, Notary or the Issuing Authority
