Several urgent questions before sending off FBR application

I have everything ready to mail off my fbr application today.

My only issues are:

1)my witness, whom is a lawyer, put his personal address and phone number. He then drew a line through these and listed his businsss address and phone. They are also show on his business card. Will this be an issue?

2)I was confused about the portion that says OR he/she certifies...that line in section E that talks about a 3rd party...is that just left blank if they know me personally and they signed the bottom?

3)is the address in top right the full address to send the application?
