Surname change should be referred as previous names ?

Hi,

I need your advice please.

I'm applying citizenship for my wife based on my Irish citizenship which I got last year. After our marriage she had changed her surname to my name (previously her surname was her father's name). Should I need to mention this in the below sections or is it only applicable if she had changed her first name

1.1) Surname, as shown in passport
1.2) Forename, as shown in passport
1.3) Previous names - Previous surname ?
1.4) Reason for changing name - Marriage?
1.5) Date of name change - New Passport issue date ?
