Re-Issue of New Indian passport.

Dear Experts .
May I plz ask about the procedure to obtain new Indian passport when your old Indian passport expires. Also to include name of my spouse in new passport.

I understand:
I need to make an appointment at first place.
I need to fill and submit and print the online registration form.
I need 52£ +7.44+2 in total as a fee.
I need my old passport original and photocopy self attested.
I need spouse's original passport and photocopy self attested.
I need marriage certificate original and photocopy.
Do I need photocopy of the valid visa in passport.?
Envelop with return address.

So I believe I go to the appointment centre with the above documents and submit then wait. Does my wife need to come with me.? Is there anything else I need to know. In UK-Edinburgh it takes 4-5 weeks.?
