Latest Cover Letter and other format needed

I have surf thro all the thread for cover letter and Maintained fund letter, however no thread is there which is updated in recent last 1-2 months for the recent updates in the Tier1 policy. I would request everyone who had submitted there application in last month to please guide us on the cover letter format and order of the document to be arranged.
