Lost Passport for Tier-1 Dependant (which form to use) ?

Lost Passport for Tier-1 Dependant (which form to use) ?

Hi Guys,

I am currently on Tier-1 General visa. I got the dependant visa for my UK born son in 2009 but I have lost his passport.

I am not sure which of the forms I need to use to get his visa again on his new passport ?

1. PBS Dependant Form
2. Transfer of Conditions Form (TOC)
