Yes you can vary (btw it is simply variation of leave and it is NOT called switching !) IHS fees will NOT be refunded I will look for a reference... but I had impression that if you apply for Tier 2 Dependent visa and vary within let say 2 weeks to ILR application... then you will get back IHS fees...
